We have a variety of activities available for each age group within Vapor Student Ministry. Listed below are links to some
of the most commonly used forms as well as a calendar of upcoming events. If you need a form that is not

Completed forms can be turned in at the Vapor Student Ministry Registration Desk (outside the Student Ministry Room) on Sunday mornings,
inserted into one of the drop boxes, mailed to or dropped off in the Church Office during the week.
Vapor Student Ministry Medical Release Form
In order for your middle school or high school age student to participate in a Vapor Student Ministry event, we must have a
notarized medical release form on file in the Church Office. Notary publics are available in the Church
Office during regular office hours.
